It is with sincere appreciation and deep regret that I write to inform you that Shoreline Wellness and Primary Care, LLC will be closing effective December 31, 2025. This decision was not made lightly. Despite our ongoing commitment to provide high-quality, patient-centered care, significant challenges related to insurance reimbursement changes, increased administrative and regulatory requirements, and overall shifts in the healthcare landscape have made it unsustainable for us to continue operating as an independent primary care practice.

For many of you, our practice has been part of your healthcare journey for years, and it has been our privilege to support your health and well-being. Please know that this closure is due solely to systemic pressures—not to any lack of commitment to you or our community.

Continuity of Care 

To ensure your care continues seamlessly, we recommend that you establish care with another primary care provider as soon as possible. If you need assistance identifying new provider options, please contact us and we will offer guidance where possible. If you are on chronic medications or have complex health needs, we encourage you to act promptly so we can help you transition your care or provide a referral.

Medical Records ​​

You may request that your records be transferred to a new provider or sent directly to you. To authorize the release of your records, please complete the Medical Records Release Form (available through clicking here, patient portal or at our office).

When requesting records, please include your full name, date of birth, and the last four digits of your Social Security number, and provide the address to which you would like the records sent. You may also request a summary rather than a full copy.

or brought to office before December 31st, 2025.

After the practice closes, medical records will be maintained at the following address:

If you do not request your records by January 31, 2026, the records will remain stored and can still be retrieved by writing to the custodian address above.

Contact After Closure 

For at least six months after closure, our office phone line will forward to a voicemail message that provides instructions on how to request medical records and how to find a new provider in case you’re unable to reach us directly. If you prefer, you may also contact the custodian by mail at the address above.

A Final Note 

Caring for you and your families has been an extraordinary honor. We are truly grateful for the trust you placed in us and for the relationships built over the years. While the healthcare environment continues to evolve, our gratitude for you remains unchanged.
